US changes passport policy
The US Consulate in Bermuda will no longer amend valid passports.
Residents requiring name changes, extension of validity for limited passports or corrections for printing errors will now have to apply for replacement passports.
If a passport needs to be changed within one year of it being issued the changes will made free of charge. The applicant will be required to submit the passport and a re-application form.
If the passport is more than one year old the applicant will be charged $67 and have to submit the old passport and a DS-82 form.
If the requested changes are due to a printing error by the State Department the customer will not be charged.

The new forms and procedures were made effective September 26 for security reasons.
